In February 2026, I conducted a Representation Assurance audit across five major AI assistants — ChatGPT, Claude, Gemini, Perplexity, and Copilot — evaluating how they represent leading U.S. healthcare insurers.
The findings were not dramatic in the traditional sense. No catastrophic errors. No obvious brand damage.
But they revealed something more important.
They revealed a governance blind spot.
A visibility gap that healthcare insurers do not currently monitor — and do not yet control.
And that gap will matter more over time.
The Two Critical Findings
The audit surfaced two consistent patterns across multiple prompts and AI platforms.
Finding 1: Inconsistent Representation of Aetna’s AI Regulatory Exposure
When asked whether Aetna had faced regulatory penalties related to AI-driven claim denials, AI assistants produced conflicting answers.
Some described federal investigations and regulatory scrutiny.
Others stated that no penalties had occurred.
Some referenced lawsuits or industry-wide controversy without clear resolution.
None of the systems produced fully consistent, clearly scoped answers.
This inconsistency does not reflect misconduct by Aetna.
It reflects something else.
It reflects the fact that AI systems are synthesizing incomplete, indirect, and inferred information to produce coherent narratives.
Those narratives may diverge significantly across platforms.
Finding 2: AI Systems Infer Internal AI Usage Where No Public Disclosure Exists
When asked which third-party AI vendors insurers use, most AI assistants confidently named vendors or described AI decision systems.
Only one assistant — Copilot — correctly identified that no specific AI vendor relationships are publicly disclosed.
This is a critical distinction.
In the absence of disclosure, AI systems do not remain silent.
They infer.
They extrapolate.
They construct plausible explanations based on industry patterns.
This is not malicious behavior. It is how modern AI systems operate.
But it creates an important new external representation surface.
Why This Does Not Cause Immediate Harm
Healthcare insurers do not rely on public AI assistants for operational decision-making.
Regulators rely on formal filings, not chatbot outputs.
Enterprise clients rely on formal contracts and due diligence.
These representation inconsistencies do not directly affect insurer operations.
Today.
But this is the wrong time horizon to evaluate the risk.
